Apply China visa if i have a valid PRC entry permit

I was born in Hong Kong. I have HK ID card and 10 year PRC entry permit. If I take China flight from Toronto to Shanghai, do I need to apply PRC visa? If so, my HK ID does not have my first name, but my Canadian passport has, can I still apply, or have to find a lawyer to sign and prove my identity.

Monita, if you have Chinese 10-year entry permit, you can use it to enter PRC, so no visa is needed any more.
