Documents required are your passport, application form, photo, return ticket, hotel reservation, previous Chinese visa and bank statement with at least 100k as well as the balance (6 months bank statement). Besides, your spouse shall execute a duly-notarized Parental Travel Permit (in a form prescribed by DSWD) certifying that he/she is allowing the minor to travel with you.
