About:China Visa Application Procedure and Fee

Thank you so much for providing answers to these questions
My mother & I are going on a Princess Cruise that starts out flying into Beijing & staying 3 days on land touring the local sights, including the Great Wall. Then, the cruise departs from Tianjin, visits Dalian, & then makes a stop in Nagasaki, Japan. After that it reenters China at Shanghai, 8 days after arriving in China, & then Hong Kong. From there, the cruise visits ports in VietNam, Thailand, & Singapore, & does not reenter China. Of course, due to political unrest or weather, ports could be drastically changed at the last moment.
My questions are:
* 1.3, 1.5, 1.6: Since these don't apply, can they be left blank?
* 1.14: I am unemployed, but basically retired, she is retired. Can I put "Other: Retired" for both?
* 2.2: Assume double or multi, since we visit Japan then return, should we increase if same cost? (Visited China previously in 2007)
* 2.4: Since it says longest intended stay in China, I assume I am to ignore the part of the journey once I INTEND to leave China, even if circumstances (such as poor health) force us to stay. So, arriving 4/1 & last China port 4/12, "12 days"?
* 2.5: Was planning to put "2011 04/01-04 Beijing; 04/04 Tianjin (embark cruise); 04/05 Dalian; 04/09 Shanghai; 04/12 Hong Kong" - Correct? Or should I list the stop in Japan midway, or the ports after Hong Kong?
* 4.7 has me stumped, I was thinking I should put the cruise company, but they aren't "in china" per se, then I thought I could put the origin hotel (Beijing Marriott City Wall) in which we will stay the first 3 days in Beijing, but they aren't involved with us after that, or the touring service within Beijing (BizTravel International Travel Service). What is standard in these cases?
* Section 5: Should we add anything here, such as the cruise itinerary?
* 6.2: Since I am filling the form out for myself & my mother, do I say "Self" & "Daughter", respectively?

*1.3,1.5,1.6: You should write N/A for these items which are not suitable for you
*1.14: yes, you can put "Other": Retired
*2.2: I'm puzzled by yoru statement, do you want to get a double or multiple entry visa? Do you think the chance of getting a double/multiple entry visa increases because you visited China in 2007?
*2.4: yes, you can write the longest intended stay duration in China is 12 days
*2.5, you should list the stop in Japan midway to prove that you need to enter a double entry visa.
*4.7: you can write the contact information of the touring service within Beijing.
*Section 5: you can write the cruise itinerary
*6.2: Relationship to the Application: You can write mother-daughter relationship

In answer to your question regarding 2.2: I read at visarite.com in their instructions for filling out the form "Note: multiple entry visa is only issued to US passport holder who have been to China and can provide a photocopy of previous Chinese visa issued in recent 5 years. First time applicant may apply for single or double entry visa." Since something could come up, such as a change in cruise itinerary or sickness which might alter my expectations of how long and when we would be in China, I thought it might come in handy to arrive there with a more lenient visa – especially if the cost of obtaining it are the same. Who knows we may receive an invitation to come back and visit later in the year.
Also, I see that I would write "daughter" when I fill out my mother's form, but when I fill out my own form, would I write "Self", something else, or leave 6.2 blank? And would I fill out 6.1, 6.3 and/or 6.4 on my own form, since it says those fields are "if this application was completed by another person..."

Ms.Leigh, you can fill the form with your request for a multiple entry. But visa officers will make the final decision. For your intended stay, a probable number is ok. For section 6, signature should be signed by applicant herself.And the form must be completed.
