How i have to write my name to my ticket application form?

My original name is in my passport Dr. Krepárt Tamás

How should I write it in the application Dr. krepárt Tamás or Dr. Krepart Tamas?

Do I hav to use my title (Dr) ?

Although I don't understand what ticket application for you need to fill in, China railway requires real-name ticketing and the name should be as same as the one in your passport, so write it Dr. krepárt Tamás
