blanks in applying VOA

I originally decided to go to HK from Japan.
After noticing S.E.Z VISA, I am planning to enter into Shenzhen in the morning via Huanggang port, and return to HK at night, so I don't stay at Hotel in Shenzhen.

When applying the Visa in Huangguang, I heard that I have to blanks in a form, but I cannot fill some of the blanks(namely, where to stay and telephone number).
Is that too suspicious to accept me?
