24 hour visa free transit eligibility

we are planning to travel from Canada to India through Shanghai with a 21 hr. layover. Do I have to fill out a form to go to a hotel outside the airport?

Yes, you will apply for 24 hour free entry permit upon your landing in Shanghai. You will also book your accommodation in advance and have a confirmation in hard copy. Since your layover is very long and if you also present your accommodation confirmation, your chances to be granted an entry permit are practically certain.
