1. your Philippines passport and a copy of the passport first page
2. fill in visa application form V.2011A and V.2011B
3. a recently-taken passport photo
4. original and a copy of your Malaysian work permit which should be valid for at least 6 months
5. round-trip air tickets, hotel booking
