I would like to ask about Chiness Visa Application Form.

You are asked to join a tour group to travel to China, which means self-service tour is not available for you. You can submit the requirements to China embassy in Kabul to apply for China visa.
Besides the visa application form, you also need to submit one recent passport photograph; a photocopy of the front, back and validity page of your passport; your passport valid for at least six months with two blank visa pages; covering letter from your company or office; original and photocopy of Invitation Letter issued by a Chinese travel agency authorized by the Ministry of Foreign Affairs of China, itinerary and name list of the tour group.
