About:Chinese Consulate in New York

Entry permit to Hong Kong

I am a citizen of Taiwan currently residing in the US. I hold a valid Tai-Bao Gen. Do I need to fill out an H-3 form? Do I have to submit and pick up the documents in person?

Yes, you need to fill out an H-3 Form, you should apply for single entry visa annotation on your Tai Bao Zheng, if you can't submit or pick up the document in person, you can ask someone else to do it on your behalf. The application by mail is not accepted.
