About:Tax Refund

tax claim with out tax form.

I have bought a product in a store here in China, but the store cannot give me the tax refund form, how can I apply for the tax refund? I have an invoice and I have a product receipt.
and my product value is more than 5 TRMB

Sorry, you can't apply for tax refund without the refund application form.
