fly from TW(A)- SH - Nanjing - HK(C) then back to TW
4/16 TW-SH (then take a train to NJ)

Yes, under the assumption that TW is Taiwan, your route is perfectly fine because Shanghai and Nanjing are located within the same 144 hours visa free transit area. All you should take care for is that your flights TW - Shanghai and NJ - HK are nonstop, i.e. that they do not have any additional landing(s) within mainland China.
