About:Folk Custom Museum

I live in Portland Oregon, where do I mail my application?

I am not sure how i will pay the cost. Do i need to send my credit car information in the application? can you elaborate more about the payment process

Michael, sorry, you cannot mail your application. You need to go to the consulate in Chicago to submit it. Payment can be done by credit card, Money Order or Cashier's Check.

Seems like the consulate in LA is a lot closer than Chicago, but I’m no map reading
