144 Hour Transit Visa...

I am Traveling from Las Vegas to Los Angeles to Shanghai on Delta Airlines. In Shanghai I will board a Cruise Ship to Japan. I already have been assured that I qualify for the 144 Hour free Transit in Shanghai, but what I need to know is where and when I will get the Paperwork that I need to fill out before I apply for this Transit Visa at the Shanghai Desk. The Delta Agent does not know what I am talking about. If someone who has done this can tell me please, do I get this in Las Vegas, when I Check In, in Los Angeles at the Desk where the Flight is Boarding, or is it Paperwork that will be given out Mid-Flight on the Trip to Shanghai? Thank you for the help.

Usually, you can fill an arrival/departure card on the airline or after your arrival at the airport.
