Studetn Visa Application Form Filling Guide?

Can someone please point me to some sample pre-filled student visa form for reference? I am facing issue in filling the following sections and am not sure what to do with them:
1) Section 2.2 - Intended Number of Entries
2) Section 2.5 - Longest intended stay in China among all entries.
3) Section 2.6 - Itinerary in China(in time sequence)

Any help is much appreciated. Thank you!

1. You should choose single entry for section 2.2
2. The intended stay depends on your study period. It may be 6 months or a year.
3. You can just state where you will go and how long you will stay for study.
