Do I need to apply Macao Visa?

I will travel to China Main Land, Hong Kong & Macao. I know I have to apply mainland China entry permit & Hong Kong entry permit. But I don't know Do I need to apply Macao entry permit.

Another one is I will do re-entry to Hong Kong during this trip. So Can I apply re-entry visa to Hong Kong.
