Tina, if you want to get her a tourist L type, you should prepare her passport and copy of its data page, the completed application form attached with a passport-style recently-taken photo, round trip air tickets and hotel reservation. Afterwards, submit the application to the consulate general of PRC to apply it for her.

Her dad lives in China and I live in the US. I will be taking her to the consulate myself. Do I need something signed by him to get the entry permit like I did the passport?

Dear, usually, a consent letter from her father may be required. But as you said, her father lives in PRC. So you’d better contact the consulate general and know about the detailed information.
