what documents are required?

My professor has invited me as a visiting scholar for a period of 3 months. Could anybody please tell me what documents I need to attached with my application form? All I've is the authorized letter sent by my professor. P.S: I lived in China for 4 years as a PhD student.

Wick, you are applying for F type, right? If so, in addition to the application form and the invitation letter from your professor, you also need to provide your passport and copy of its data page, an invitation letter of a duly authorized unit, a supporting letter from the organization you serve in, your bank statement (the last six months).
