If you will get married in PRC, you don’t need to present medical certification. You need to provide your passport, your entry permit to PRC, single status certificate. You can apply for a tourist L type to enter PRC and then get married.
As for L type application, the following documents are required including your passport and its copy, the application form attached with a recently-taken passport styled photo, round trip air tickets and hotel reservations, or an invitation letter from an individual in PRC. As for your situation, you can ask your boyfriend issue an invitation letter to you. Afterwards, submit your application to the embassy of PRC and wait for the result. If it’s the first time you apply for PRC entry permit, you also need to provide the financial support proof such as bank savings account, BIR-stamped income tax return form or income certificate.
