Hello, Cindy, the documents include passport and its copy, the application form, a recently-taken bareheaded color photo, travel itinerary, round trip air tickets and hotel reservation.
If the minor travels alone, he/she should provide Travel Clearance issued by the Department of Social Welfare and Development.
If the minor travels with one of his/her parents, the parent who will be left behind should fill out a duly-notarized Parental Travel Permit (a form from DSWD) certifying that he/she allows the minor to travel with his/her spouse.
A Travel Clearance is not required if the minor is traveling with both his/her parents.
