1. a valid passport and its copy
2. a completed application form attached with your recent color bare-headed photo
3. the original and photocopy of your residence permit in Vietnam
4. the original and photocopy of your admission letter
5. the original and photocopy of JW 201 or 202 form

so what can stand for resident permit. I guess if I have a valid vietnam visa and not expired yet should be enough.
