How to apply for a new passport?

I am a Chinese, 75 years old, living in Oregon.
My passort is expired.
(I have no green card, no work)
What document do I need to appy for a new passport.

Hey Lily! The consular office may ask you to provide the documents below:
1. A completely and properly filled application form;
2. Four full-face 48mmx33mm color bare-headed photos against white background;
3. The original and photocopy of your passport;
4. The original and photocopy of your US visa;
5. A personal statement containing a brief introduction to yourself and the reason for renewing the passport.
