Residence registration recovery

Hi, I want to extend my tourist visa, but I have lost the paper the police gave to me when I registered my residence. How may I recover it? Are there additional documents required for recovering it from the police?

German, I think you should visit local police station where you got the registration form as soon as you can. Please try to ask them if you can apply for a new one.
