Will a letter from a doctor saying I am healthy suffice to get a Z Visa or is there some form needed

Do I actually need any document at all about health to obtain the Z entry permit in the US? I don't see it listed as a requirement but have seen some comments in reference to a medical form.

Louise, before you apply for the Z type entry permit, you should apply for the work permit in China. During the application for the work permit, you should provide a medical report.
