Do you think i will need any other documents to apply a tourist visa for China from Cyprus?

As an EU citizen i do not need any residence permit to stay in Cyprus.I just have my passport showing my legal status in Cyprus. I trie to download a form from internet and fill it up for my application.But it was not possible. Do i need to come to the embassy and fill in a form there?
