Visa Form and multiple entry qualification

Sir, I planned to visit china from Dec 17th to Dec24th. This is my first travel to china. I am Engineering consultant and bringing machinery from China. Already I installed one plant worth around USD 75,000. This is second order and want to approve the machine before dispatch to Lanka. I had invitation letter of duly authorized agent.

What application form should I fill. There are two forms available namely V2011A and V 2013.

As I am dealing with china machinery, often I need to travel to china. Can I get the multiple entry for one year. Please guide me.
