Travel documents for a new born

My wife is Chinese and I am Canadian, both working and living in Shanghai. We will have our baby born in the USA close to Santa Barbara because some medical issues are involved with her pregnancy. Once the baby is born, do we need to go to Los Angeles to get a travel document for the baby since he will be a USA citizen by birth and Canadian?
Is the form of this travel document online?

Yes, you should apply for the travel document for your baby if you want to take him/her back to PRC. The form can be downloaded from the Chinese consulate in Los Angelos. But it is written in Chinese. By the way, you, your wife and your baby should present at the conulate when applying for the travel document. If you or your wife can't come, a written announcement letter from her is needed and it shouldbe notarized too.

what is a written announcement letter from my wife to be presented to the consulate. How does it look like since it has to be notorized?

Sorry, the consulate doesn't specify the sample. It just says that a notarized letter has to be signed by your baby's mom. The content is that she agrees to let you apply for the travel document for your baby.

I think you should go to the consulate or call them for details. Sorry, the Chinese consulate in Los Angeles doesn't provide the phone number.
