student visa on a exchange program

Question

I am currently studying in an American University but a Sri Lankan National. I have been accepted to study in a Chinese University for three month. Fees and lodgings fully paid by my American University. In order to apply for student X, what documents do I have to submit, other than appliaction form.

If you are going to study there, JW202 Form and Admission Notice issued by the university are required for the application. Besides, you should prepare an foreigner's physical examination record for application.
