Stolen passport

A Chinese citizen had his passport stolen in California.
He lives in Cincinnati.
How does he get a new passport for himself and his 6 year old daughter?

He should apply in person for a new passport from Chinese consulate in New York with following documents:
1. Fill the application form for passport application;
2. 4 recent pasport-sized photos, 48mmx33mm with white background;
3. Copy of passport if any, or other document showing Chinese citizenship such as ID card and birth certificate;
4. Passport loss report, with detailed information of the loss, and personal information, passport numner&validity, issue place&date, US visa or residence permit (original and copy).
