Does tourist visa need Physical Examination Record for Foreigner?

Hi, I am applying for a "L" tourist, single entry. I am Swiss and applying for it from Switzerland. Do I need to get a "Physical Examination Record for Foreigner"?

No, it is not necessary for short-term visitors (for a stay less than 6 months).
