I wish to visit on December, what other requirements needed aside from application form?

You should provide:
1. passport valid at least six months before its expiration and copy of its information page.
2. a recent passport size photo affixed to the application form.
3. an invitation letter from Chinese authorized travel agencies or Chinese citizens or your foreign friends there. If you can't provide this letter, you should offer round trip air tickets and hotel booking reservation.
4. If you visit for the first time, you should provide personal financial statement such as recent 6 months deposit proof and BIR-stamped income tax return form.
