About:Chinese Consulate in Los Angeles

What visa is needed for us passport going on a cruise stopping in Shanghi and Beijing for 4 days

What application form do we need and must we go in person to the China Consulate or can we do everything by mail.

You need a double entry China tourist visa. Mail application is not acceptable. You can entrust others to hangle your application if you can not visit in person.
