Visa needed

Hi i understand that since i am visiting beijing from 9nov to 1st dec , i need a visa right? in the form under
2.5, i need to indicate my residence n phone no during my stay?do i need to fill up every single day of my bookings
cos i have yet to finalise my cities or hotels i intend to visit, can i indicate maybe a few?is ok?

as for friend or relative or inviter under 2.8 , if dont have? i can leave it blank,right. thanks so much for your help in this.
