VOA for emergency situation

Hi, my grandfather is in critical condition; my mom is a Canadian citizen and wants to apply for the visa on arrival, what document does she need for this matter? We would prefer her to take the flight on Sunday 09/16/2012 and arrives in Hongkong then enter China through Shenzhen. Any help is much appreciated.
