Visa and V.2011B forms for China

I am a south african living in the UK. I will be leaving to Shanghai in June-July with my university. They gave me some forms to fill which are : the visa application form and a V.2011B form(Is this form applicable to me?). Are there any other forms that I need to fill in? or any other information that I need to know/provide?

Since you are applying for a China visa in a third country other than your home country, you shoudl fill and submit V.2011B form. As an alian in UK, you also need to provide proof of your residence status in UK to get a China visa such as residence permit, work or study visa, water or electricity bills or tenancy agreement or a valid U.S. visa.
