HK Scheme 6000

I was born in Hong Kong and would like to apply for a permanent HK ID card to request for the scheme 6000 money from Hong Kong. I have my HK birth certificate. Do I go to the China Consulate in Vancouver on Granville Street to apply for the permanent HK ID or can I complete the application online and then bring it for processing at the Granville street office?

If I need to go to the Granville St. office to apply in person, then do I need to make an appt. or do I just walk in. I hear you need to bring in your HK birth certificate and your marrigage certificate. Is there anything I need to bring with me? Thanks.
