You are eligible to apply for a Home Return Card (Home Visit Permit) for China. However, you can't get it in Belgium, please go to Hong Kong and submit the application to China Travel Service Hong Kong.

No, you don't need to give up your Belgium passport and replace it with a Hong Kong passport. You just need to submit the following documents to the China Travel Service Hong Kong:
1. Hong Kong and Macau Residents to and from Mainland China Permit Application Form
2. one recent color photo (more than 40mm×50mm,less than 45mm×55mm)
3. HK permanent Identity card and its copy
4. fees
