The visa issue in Singapore is handled by the CVASC on behalf of the embassy. Add: 22 Malacca Street, #09-00 Royal Brothers Building.
The requiremetns include: your maid's valid passport, Singapore resident permit, visa application form V2011A and V2011B, passport photo, round-trip air ticket and hotel reservation.
The visa fee is SGD 50 (single-entry visa fee) + SGD 25 (tax inclusive) (regular service fee for the CVASC).
