what are the documents required for z visa application?

pls let me know what are the documents i need to submit with the invitation letter for Z visa from Bangladesh.Is there any possibility that i will not get the visa even after i submit all the required documents.Bcz a person wrote the other day he was denied a z visa even after submitting all the documents.Pls advise can it happen like this.Thanks in advance for the reply
