Yes, you need a visa to Macau, but you can apply for a visa on arrival.

We have a client, a vatican passport holder, he wishes to go to Macau by Ferry from Hongkong for a visit.

If he is allowed visa on arrival in Macau, what documents does he need to present? ALso, does he need visa to Hong Kong as well?

He does not need a visa to Hong Kong for not more than 14 days. For a macao visa, he needs to apply in advance. The detail requirements information are listed above on this page, so better take a look as reference.
