Mr.jaz, as I know, the passport number should be included in the affidavit, which is an important information. I made the AFFIDAVIT OF SINGLE STATUS in USA, its format is:

BEING DULY SWORN DEPOSE AND SAY:

However, I don't know whether the format of affidavit in India is similar to USA or not.
