I would like to know some answer. First question, I am a candidate of Doctor's degree from Sichuan University, Chengdu, China. I am Srilankan. I have to attend one conference at Hawaii. So, what I should be apply what type of visa application. And I have not found DS-160 form and cannot open online visa website. So, how can I get visa form. If I can take at Consulate General of U.S.A, Chengdu, I will be comfortable. Second question, how many photos need for visa application. Thanks alot...
