About:Chinese Consulate in New York

Anybody Help would be greatly appreciated. Please Anyone from the State Of PA.

I live in the State Of Pennsylvania, USA. Dealing with the authentication process for Marriage steps for china. Was told I need to get form Affidavit Of Single
Status form filled and get Notarized, this is complete. Than have to send to "Secretary Of My State" to get Authenticated, this is completed . Now have to send these forms to
get Authenticated at "Consulate-General Of The Peoples Public Of China in New York".

My questions are: (1.) Do I need my original Passport sent to them Or Just A Photo Copy? (2). Is the Notarized & The Secretary Of State Authenticated form Stapled together considered One Form , or two? So I know what to pay? (3.) On the (G-1) form that has to be filled out. In the area of: Content of each Document.Please Submit Original Documents and Photocopies, 1 thru 5 . Does "Affidavit Of Single Status" & "Passport" get filled in there?

(1)the Photocopy of your passport is reequired for the authentication application at the consulate of China in New York
(2)Notarized & the Secretary of State Authentication Form stampled together is considered as one Form. The fee is 20 dollars.
(3)you just need to fill the Affidavit of Single Status on the G-1 Form, the passport is not needed because you just apply for authentication of the Affidavit of Single Status
