Also, I'm going to get my letters of invitation from relatives faxed to your office. Could you please hold them until I arrive on Monday morning, 20th July.Thanks

You have to fax a written request to the embassy at 02-62739615 to ask for the form.You'd better submit all the documents in person (by yourself or someone else, namely your friends, relatives or travel agents) due to the likely risks of documents' safety.
