Queries Regarding Application for Five Year Tourist Visa

Hi
I am currently applying for a five year L-Tourist visa and I have some questions regarding entries on the application form. Please can you advise.
1) Sections 1.15 and 1.16 request the names of my parents and their address. Both my parents are deceased, so should I put 'deceased' in the address field or enter their address at date of death? Alternatively, as they are no longer living, should I tick the 'not applicable' box and not enter any information about my parents?
2) Section 1.8 requests 'Place of Issue' for my passport. UK passports do not include a place of issue, but instead list the issuing authority with no location. What answer should I enter?
3) Section 2.4 under 'Date of Departure from China' requests 'Arrival train/ship/flight No'. Is this supposed to be 'Departure train/ship/flight No'? In other words, should I enter the flight on which I arrived into China or the one on which I am departing?
4) I have had two previous 2 year L visas, the last of which expired in November 2021. I wish to now apply for the new five year multiple entry visa, but I can't see anywhere on the application form to request that option. How do I do request the five year visa?

1. You can put deceased in.
2. You may fill in the issuing authority.
3. You need to fill in the one on which you are departing.
4. The duration and validity of the visa is decided by the consular office. You can tell the staff at the visa center your expected one.

Hi Lori
Thanks for your help. I have one more question. On the form it asks whether I have ever had my fingerprints taken for previous Chinese visas. I have had several visas over the last few years, the most recent of which was issued in November 2019. All have been issued in London.
I think I may have had my fingerprints taken on one occasion, but I'm not absolutely certain. Even if I did have them taken, I have renewed my passport recently and the visas were in an earlier one with a different number.
1) Should I answer the question 'Yes' or, as I'm unsure, is it better to assume I haven't had my fingerprints done in the past and to answer 'No'?
2) Will the visa centre check their records and correct my application form if it is wrong?
3) If it turns out that I have already provided fingerprints, do I still need to attend the visa office with my application or can it then be done by post?

I'm afraid I have another question.
I'm confused about the photo requirements. I have uploaded a photograph and it was accepted during the online application. Is that all I need to do or must I also provide a printed version of the same photograph when attending the appointment at the visa centre?

1. The requirement about the fingerprints came out in 2018 so I'm pretty sure that your fingerprints were collected during the previous application. So you can just answer Yes.
2. They may have a quick check and for small mistakes you may correct them at the visa center but not all visa centers give you the opportunity to correct. So it's advised to check the information before submission.
3. Yes, you still need to attend the visa center to submit in person.

You need a printed version of the same photograph when attending the visa center.
