Will my visa be rejected if not every field on the COVA form is in block capital lettering? And if I add more information by hand (but do not correct or change information), is that also grounds for the visa to be rejected?

You cannot add information by hand and all the information should be filled online then printed out and signed. And you need to ensure that every word is spelled correctly.
