Documents you need:
Passport
Visa Application Form (filled in online and printed out) and Photo
Proof of legal stay or residence status in Japan
Documents showing the itinerary including air ticket booking record (round trip) and proof of a hotel reservation
You don't need an appointment. You shall go to the CVASC to submit the application. Tokyo, Osaka and Nagoya all have the CVASC. You can choose the nearest one.
