I have a 12 hours layover in Shanghai. Do I need to fill out Health Declaration form and do a COVID-19 test?

Yes, you do. You will make DIY test and take a photo of the negative result on your smartphone. Also, you will fill in a Chinese Health Declaration Form. This is so rarely checked, but you should perform this just for your peace of mind.
