COVID entry requirements

I am travelling from London - Guangzhou - Ho Chi Minh City.

Do I need to fill in the health declaration form for a transit flight in Guangzhou from the UK? I'm staying there for 3 hours

Yes, you are supposed to do that, although more important detail is that you perform DIY test and bring a photo of your negative test. It is quite probable that nobody will ask about it, but just do it to fulfill the requirement.
