Which Birth Certificate Form need for Canadian citizen to visit relatives in China?

My child age 17 was born in Canada, and will visit her Chinese cousin and aunt in China for 1 month next summer. She has a Canadian passport. Which form of the birth certificate does she need to apply for Q2 Visa, the long form with parents information or the short form without parents info? Thank you.

As far as I know, birth certificate is not required for applying for Q2 visa.
