Does visa application for Family have to use individual application form?

I will travel to mainland China end of December with 2 kids. We are iNdonesian passport holders but living in Singapore. Should I use individual application form when applying a visa or i can add my kids information in my form (both of them are below 18 years). Is there any specific document/form to fill in for under 18?

If the two kids have their own passports, you need to complete separare application forms for them. You may need to take their birth certificates, passports of parents, and authorization letter from the parent who does not travel together.
